Take notice that:

- Pursuant to subsection 65.11(1) of the Act, the debtor gives you notice of his/her intention to disclaim or resiliate the following agreement (provide sufficient details of the agreement to enable it to be identified):
- Pursuant to subsection 65.11(3) of the Act, within 15 days after the date on which this notice is given, any party to the agreement may, with notice to the other parties to the agreement and the trustee, apply to the Court for an order that the agreement is not to be disclaimed or resiliated.
