This section of the website provides access to Corporations Canada forms that relate to not-for-profit organizations, in PDF format. The forms can be printed then completed manually, or completed on screen then printed. The filled-out form can be sent to Corporations Canada by fax or mail.
An annex page has been provided should you need additional spaces for director names and addresses. Use as many as required.
