Instructions for Form 3022 – Annual Return

You are providing information required by the Canada Cooperatives Act (Coop Act). Note that both the Coop Act and the Privacy Act allow this information to be disclosed to the public. It will be stored in personal information bank number IC/PPU-049.

Item 1: Name of the cooperative

Indicate the name of the cooperative. You can verify the exact name of the cooperative in its articles and in Search for a Federal Corporation.

Item 2: Cooperative number

Indicate the cooperative number. You can verify the cooperative number on your certificate of incorporation, amalgamation or continuance and in Search for a Federal Corporation.

Item 3: Year of filing

Indicate the year for which the annual return is being filed. The information provided must be as of the anniversary date for the year for which the annual return is being filed.

Note

The anniversary date is the date the cooperative was created or the date the cooperative first came under Coop Act (for example, the date of incorporation, continuance or amalgamation). The date is generally found on the cooperative's certificate of incorporation, amalgamation or continuance. You can also Search for a Federal Corporation.

Item 4: Date of last annual meeting

Indicate the date of the last annual meeting or the date of the last written resolution in lieu of a meeting. A written resolution is a written record of decisions that is signed by all of the persons who are entitled to vote.

Item 5: Does the cooperative distribute its securities to the public?

Indicate if the cooperative distributes its securities to the public.

Item 6: Declaration

This form must be signed by an individual who has relevant knowledge of the cooperative and who is authorized by the directors. This includes:

  • a director of the cooperative
  • an authorized officer of the cooperative
  • an authorized agent.

Print the name of the signatory.

Filing fees

You must include the filing fee for cooperatives.
